包皮過長

病患者男 - 46歲

醫生你好，本人有包皮過長問題，勃起時龜頭不能完全自動露出，要手動拉動包皮向後才能露出龜頭，過程沒有痛楚，進行性行為時也沒有問題，包皮拉向後會有些少褶皺，亦從未試過發炎

請問這是否假性包皮過長？
任何病痛也沒有，只是有2, 3 次被伴侶投訴有少少味，但本人完全無發現這問題，是否仍有需要割包皮？
4x歲是否仍需要做割包皮？
